Who are You?

You are an experienced health professional with a customer service approach.  You are an effective communicator with strong problem solving skills and an ability to work well as part of a diverse team, including residents and families. You have experience in caring for people with dementia, as well as residents at end of life. You are familiar with the Nova Scotia Long Term Care Standards and the licensing process, and are comfortable navigating digital platforms and tools.  

Job Summary:

Reporting to the Director of Quality and Professional Practice, the Care Navigator is responsible for bed management, including reviewing and overseeing all resident admissions and internal transfers.  The Care Navigator is the liaison for Continuing Care and acts as a resident and family resource.  The Care Navigator is also closely involved in the Palliative Care team, among others, and organizes and facilitates Resident Care Conferences in compliance with LTC standards.

Qualifications:

  • Licensed Nova Scotia health care professional
  • Minimum of three (3) years experience in health care
  • Long Term Care experience an asset
  • Highly organized
  • Skilled in working with a multidisciplinary team
  • Excellent communication skills with residents, families, coworkers, and external agencies
  • Proficiency with computer use, particularly MS Office and Electronic Health Records

Benefits: We offer a comprehensive compensation package including an attractive benefits plan, defined benefit pension and a competitive salary

Hours of Work:  80 hours bi-weekly.  The regular hours of work are intended to be from 0800-1600, Monday through Friday.  Given the scope and responsibility of the role, there may be occasions where the Care Navigator may be required to stay beyond this time period.
