Moving In

Moving in to long term care, or helping a loved one move into care, can be a stressful and confusing process. A move into long term care is facilitated through Single Entry Access. All prospective clients are required to make application by calling the Continuing Care Office. In short, an assessment will be conducted in the person’s home or in hospital by a Care Coordinator from the Department of Health. The Care Coordinator will also help people start the financial process. Once all assessments have been completed and approved, the applicant’s name will be added to the wait lists for the home of preference. These wait lists are centrally managed by a placement coordinator in the Department of Health.
