A move into long term care is facilitated through the province. The toll free number is 1-800-225-7225, and you can also gain access to lots of valuable information on their website at: https://novascotia.ca/dhw/ccs/long-term-care.asp
In short, an assessment will be conducted in the person’s home or in hospital by a Care Coordinator from the Department of Health. A Care Coordinator will also help start the financial process. Once all assessments have been completed and approved, the applicant’s name will be added to the wait lists for the home of preference. These wait lists are centrally managed by a placement coordinator in the Department of Health.
